District 2-T1 Eye Bank/Surgery Program
Our Eye Bank/Surgery Program is divided into four regions in the top 26 counties of the Texas Panhandle.
If you need sight saving surgery, locate the county and district in which you live below:
District 1: Dallham, Sherman, Hansford, Ochiltree, Lipscomb and Moore.
District 2: Hartley, Hutchinson, Roberts, Hemphill, Carson, Gray, Wheeler, Donley Collingsworth and Childress.
District 3: Oldham Deaf Smith, Armstrong, Parmer, Castro, Swisher, Brisco and Hall.
District 4: Potter and Randall.
Then contact one of the directors in that district listed below. The director will help you fill out and submit the application for sight saving surgery.
